Pathogenic for Hereditary breast ovarian cancer syndrome — the classification assigned by Labcorp Genetics (formerly Invitae), Labcorp to NC_000017.10:g.(?_41199654)_(41203140_?)del, citing Invitae Variant Classification Sherloc (09022015): This variant is a gross deletion of the genomic region encompassing exon(s) 20-22 of the BRCA1 gene, which includes the termination codon. This deletion extends beyond the assayed region for this gene and therefore may encompass additional genes. While this deletion is not anticipated to lead to nonsense mediated decay, it is expected to alter mRNA translation or result in a truncated protein product. A similar copy number variant has been observed in individual(s) with breast and/or ovarian cancer (PMID: 15863663). This variant is also known as deletion of exons 21-23 by alternative exon numbering. This variant disrupts the C-terminal end of the BRCA1 protein partially including the BRCT domain (residues 1646-1859), which is important for DNA repair activity (PMID: 11573086, 14576433, 15133503, 25652403). While functional studies have not been performed to directly test the effect on BRCA1 protein function, this suggests that disruption of the C-terminal portion of the protein is functionally important. This variant disrupts a region of the BRCA1 protein in which other variant(s) (p.Gly1788Val) have been determined to be pathogenic (PMID: 9796975, 14534301, 15689452, 18512148, 20516115, 21614564, 27272900). This suggests that this is a clinically significant region of the protein, and that variants that disrupt it are likely to be disease-causing. For these reasons, this variant has been classified as Pathogenic.
