Pathogenic — the classification assigned by Labcorp Genetics (formerly Invitae), Labcorp to NM_033380.3(COL4A5):c.3604+1G>A, citing Invitae Variant Classification Sherloc (09022015): This sequence change affects a donor splice site in intron 40 of the COL4A5 gene. RNA analysis indicates that disruption of this splice site induces altered splicing and likely results in a shortened protein product. This variant is not present in population databases (gnomAD no frequency). Disruption of this splice site has been observed in individual(s) with Alport syndrome (PMID: 2995919). Studies have shown that disruption of this splice site results in skipping of exon 40, but is expected to preserve the integrity of the reading-frame (PMID: 29959198). This variant disrupts the triple helix domain of COL4A5. Glycine residues within the Gly-Xaa-Yaa repeats of the triple helix domain are required for the structure and stability of fibrillar collagens (PMID: 7695699, 8218237, 19344236). In COL4A5, missense variants at these glycine residues are significantly enriched in individuals with disease (PMID: 23720012, 27627812) compared to the general population (ExAC). For these reasons, this variant has been classified as Pathogenic.
