Uncertain significance for Lymphoproliferative syndrome 1 — the classification assigned by Labcorp Genetics (formerly Invitae), Labcorp to NM_005546.4(ITK):c.85C>T (p.Arg29Cys), citing Invitae Variant Classification Sherloc (09022015): This sequence change replaces arginine, which is basic and polar, with cysteine, which is neutral and slightly polar, at codon 29 of the ITK protein (p.Arg29Cys). This variant is not present in population databases (gnomAD no frequency). This missense change has been observed in individual(s) with primary hemophagocytic lymphohistiocytosis (PMID: 31388699). ClinVar contains an entry for this variant (Variation ID: 2417920). Invitae Evidence Modeling of protein sequence and biophysical properties (such as structural, functional, and spatial information, amino acid conservation, physicochemical variation, residue mobility, and thermodynamic stability) indicates that this missense variant is not expected to disrupt ITK protein function with a negative predictive value of 95%. Experimental studies have shown that this missense change affects ITK function (PMID: 17412921, 19535334, 19701889). In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance.
