Uncertain significance for Hypertrophic cardiomyopathy 1 — the classification assigned by Labcorp Genetics (formerly Invitae), Labcorp to NM_033118.4(MYLK2):c.403G>A (p.Gly135Ser), citing Invitae Variant Classification Sherloc (09022015). This variant lies in the MYLK2 gene (transcript NM_033118.4) at coding-DNA position 403, where G is replaced by A; at the protein level this means replaces glycine at residue 135 with serine — a missense variant. Submitter rationale: This sequence change replaces glycine with serine at codon 135 of the MYLK2 protein (p.Gly135Ser). The glycine residue is highly conserved and there is a small physicochemical difference between glycine and serine. This variant is not present in population databases (ExAC no frequency) and has not been reported in the literature in individuals with a MYLK2-related disease. Algorithms developed to predict the effect of missense changes on protein structure and function (SIFT, PolyPhen-2, Align-GVGD) all suggest that this variant is likely to be tolerated, but these predictions have not been confirmed by published functional studies. In summary, this is a novel missense change that is not predicted to affect protein function or cause disease. However, the evidence is insufficient at this time to prove that conclusively. It has been classified as a Variant of Uncertain Significance.
