Uncertain significance for Familial cancer of breast — the classification assigned by Labcorp Genetics (formerly Invitae), Labcorp to NM_024675.4(PALB2):c.1422T>G (p.Ser474Arg), citing Invitae Variant Classification Sherloc (09022015): This sequence change replaces serine with arginine at codon 474 of the PALB2 protein (p.Ser474Arg). The serine residue is highly conserved and there is a moderate physicochemical difference between serine and arginine. This variant is not present in population databases (ExAC no frequency) and has not been reported in the literature. In summary, this is a novel missense change with uncertain impact on protein function and mRNA splicing. It has been classified as a Variant of Uncertain Significance. Algorithms developed to predict the effect of missense changes on protein structure and function do not agree on the potential impact of this missense change (SIFT: "Deleterious"; PolyPhen-2: "Probably Damaging"; Align-GVGD: "Class C0"). Algorithms developed to predict the effect of nucleotide changes on mRNA splicing suggest that this variant may alter mRNA splicing, but this prediction has not been confirmed by published transcriptional studies.
