Uncertain significance — the classification assigned by Women's Health and Genetics/Laboratory Corporation of America, LabCorp to NM_006231.4(POLE):c.5726G>A (p.Arg1909Gln), citing LabCorp Variant Classification Summary - May 2015. This variant lies in the POLE gene (transcript NM_006231.4) at coding-DNA position 5726, where G is replaced by A; at the protein level this means replaces arginine at residue 1909 with glutamine — a missense variant. Submitter rationale: Variant summary: POLE c.5726G>A (p.Arg1909Gln) results in a conservative amino acid change located in the C-terminal domain (IPR013697) of the encoded protein sequence. Four of five in-silico tools predict a benign effect of the variant on protein function. The variant allele was found at a frequency of 4.8e-05 in 251294 control chromosomes (gnomAD). The observed variant frequency is approximately 3 fold of the estimated maximal expected allele frequency for a pathogenic variant in POLE causing Colorectal Cancer phenotype (1.4e-05), suggesting that the variant may be benign. To our knowledge, no occurrence of c.5726G>A in individuals affected with Colorectal Cancer and no experimental evidence demonstrating its impact on protein function have been reported. Two ClinVar submitters (evaluation after 2014) cite the variant as uncertain significance. Based on the evidence outlined above, the variant was classified as VUS possibly benign.
