This variant lies in the ALK gene (transcript NM_004304.5) at coding-DNA position 4275, where G is replaced by A; at the protein level this means the protein sequence is unchanged (leucine at residue 1425 retained) — a synonymous variant. Submitter rationale: The ALK c.4275G>A (p.Leu1425=) synonymous change has a maximum subpopulation frequency of 0.01% in gnomAD v2.1.1 (https://gnomad.broadinstitute.org/). Algorithms that predict the impact of sequence changes on splicing indicate that this change may impact splicing, but to our knowledge these predictions have not been confirmed by functional studies and our internal data based on RNA-seq analysis of a tumor sample of the patient is indeterminate (see Comments). To our knowledge, this variant has not been reported in the literature in individuals with ALK-related neuroblastic tumor susceptibility. In summary, the evidence currently available is insufficient to determine the clinical significance of this variant. It has therefore been classified as of uncertain significance.
