NM_004281.4(BAG3):c.784G>A (p.Ala262Thr) was classified as Uncertain significance for Cardiovascular phenotype by Ambry Genetics, citing Ambry Variant Classification Scheme 2023. This variant lies in the BAG3 gene (transcript NM_004281.4) at coding-DNA position 784, where G is replaced by A; at the protein level this means replaces alanine at residue 262 with threonine — a missense variant. Submitter rationale: The p.A262T variant (also known as c.784G>A), located in coding exon 3 of the BAG3 gene, results from a G to A substitution at nucleotide position 784. The alanine at codon 262 is replaced by threonine, an amino acid with similar properties. This variant has been detected in brothers affected with dilated cardiomyopathy (DCM) and who were suspected to have an additional alteration in BAG3 identified (Norton N et al. Am. J. Hum. Genet., 2011 Mar;88:273-82). This amino acid position is well conserved in available vertebrate species. In addition, the in silico prediction for this alteration is inconclusive. Since supporting evidence is limited at this time, the clinical significance of this alteration remains unclear.
