Likely pathogenic for Hereditary cancer-predisposing syndrome — the classification assigned by Ambry Genetics to NM_000535.7(PMS2):c.705G>A (p.Gln235=), citing Ambry Variant Classification Scheme 2023: The c.705G>A variant (also known as p.Q235Q) is located in coding exon 6 of the PMS2 gene. This variant results from a G to A substitution at nucleotide position 705. This nucleotide substitution does not change the amino acid at codon 235. However, this change occurs in the last base pair of coding exon 6, which makes it likely to have some effect on normal mRNA splicing. This variant has been identified in proband(s) whose Lynch syndrome-associated tumor demonstrated loss of PMS2 expression by immunohistochemistry (Li S et al. J. Med. Genet. 2020 Jan;57:62-69; Ambry internal data). This nucleotide position is highly conserved in available vertebrate species. In silico splice site analysis predicts that this alteration will weaken the native splice donor site. This variant is considered to be rare based on population cohorts in the Genome Aggregation Database (gnomAD). Based on the majority of available evidence to date, this variant is likely to be pathogenic.
