NM_000051.4(ATM):c.28A>G (p.Ile10Val) was classified as Uncertain significance for Hereditary cancer-predisposing syndrome by Ambry Genetics, citing Ambry Variant Classification Scheme 2023. This variant lies in the ATM gene (transcript NM_000051.4) at coding-DNA position 28, where A is replaced by G; at the protein level this means replaces isoleucine at residue 10 with valine — a missense variant. Submitter rationale: The p.I10V variant (also known as c.28A>G), located in coding exon 1 of the ATM gene, results from an A to G substitution at nucleotide position 28. The isoleucine at codon 10 is replaced by valine, an amino acid with highly similar properties. This variant was not reported in population based cohorts in the following databases: Database of Single Nucleotide Polymorphisms (dbSNP), NHLBI Exome Sequencing Project (ESP), and 1000 Genomes Project. In the ESP, this variant was not observed in 6498 samples (12996 alleles) with coverage at this position. To date, this alteration has been detected with an allele frequency of approximately 0.002% (greater than 66000 alleles tested) in our clinical cohort. This amino acid position is not well conserved in available vertebrate species. In addition, this alteration is predicted to be tolerated by in silico analysis. Since supporting evidence is limited at this time, the clinical significance of p.I10V remains unclear.
