Uncertain significance for Hereditary cancer-predisposing syndrome — the classification assigned by Ambry Genetics to NM_000075.4(CDK4):c.728C>A (p.Ser243Tyr), citing Ambry Variant Classification Scheme 2023. This variant lies in the CDK4 gene (transcript NM_000075.4) at coding-DNA position 728, where C is replaced by A; at the protein level this means replaces serine at residue 243 with tyrosine — a missense variant. Submitter rationale: The p.S243Y variant (also known as c.728C>A), located in coding exon 6 of the CDK4 gene, results from a C to A substitution at nucleotide position 728. The serine at codon 243 is replaced by tyrosine, an amino acid with dissimilar properties. This variant was not reported in population based cohorts in the following databases: Database of Single Nucleotide Polymorphisms (dbSNP), NHLBI Exome Sequencing Project (ESP), and 1000 Genomes Project. In the ESP, this variant was not observed in 6503 samples (13006 alleles) with coverage at this position. To date, this alteration has been detected with an allele frequency of approximately 0.01% (greater than 17000 alleles tested) in our clinical cohort. This amino acid position is well conserved in available vertebrate species. In addition, this alteration is predicted to be tolerated by in silico analysis. Since supporting evidence is limited at this time, the clinical significance of p.S243Y remains unclear.
