Uncertain significance for Hereditary cancer-predisposing syndrome — the classification assigned by Ambry Genetics to NM_000179.3(MSH6):c.452A>T (p.Tyr151Phe), citing Ambry Variant Classification Scheme 2023. This variant lies in the MSH6 gene (transcript NM_000179.3) at coding-DNA position 452, where A is replaced by T; at the protein level this means replaces tyrosine at residue 151 with phenylalanine — a missense variant. Submitter rationale: The p.Y151F variant (also known as c.452A>T), located in coding exon 2 of the MSH6 gene, results from an A to T substitution at nucleotide position 452. The tyrosine at codon 151 is replaced by phenylalanine, an amino acid with highly similar properties. This variant was not reported in population based cohorts in the following databases: Database of Single Nucleotide Polymorphisms (dbSNP), NHLBI Exome Sequencing Project (ESP), and 1000 Genomes Project. In the ESP, this variant was not observed in 6503 samples (13006 alleles) with coverage at this position. To date, this alteration has been detected with an allele frequency of approximately 0.002% (greater than 65000 alleles tested) in our clinical cohort. This amino acid position is well conserved in available vertebrate species. In addition, this alteration is predicted to be tolerated by in silico analysis. Since supporting evidence is limited at this time, the clinical significance of p.Y151F remains unclear.
