Uncertain significance for Hereditary cancer-predisposing syndrome — the classification assigned by Ambry Genetics to NM_002382.5(MAX):c.269G>T (p.Arg90Leu), citing Ambry Variant Classification Scheme 2023. This variant lies in the MAX gene (transcript NM_002382.5) at coding-DNA position 269, where G is replaced by T; at the protein level this means replaces arginine at residue 90 with leucine — a missense variant. Submitter rationale: The p.R90L variant (also known as c.269G>T), located in coding exon 4 of the MAX gene, results from a G to T substitution at nucleotide position 269. The arginine at codon 90 is replaced by leucine, an amino acid with dissimilar properties. This variant was not reported in population based cohorts in the following databases: Database of Single Nucleotide Polymorphisms (dbSNP), NHLBI Exome Sequencing Project (ESP), and 1000 Genomes Project. In the ESP, this variant was not observed in 6503 samples (13006 alleles) with coverage at this position. To date, this alteration has been detected with an allele frequency of approximately 0.02% (greater than 4500 alleles tested) in our clinical cohort. This amino acid position is highly conserved in available vertebrate species. In addition, this alteration is predicted to be benign and deleterious by PolyPhen and SIFT in silico analyses, respectively. Since supporting evidence is limited at this time, the clinical significance of p.R90L remains unclear.
