Uncertain significance for Hereditary cancer-predisposing syndrome — the classification assigned by Ambry Genetics to NM_007294.4(BRCA1):c.1853G>A (p.Arg618Lys), citing Ambry Variant Classification Scheme 2023. This variant lies in the BRCA1 gene (transcript NM_007294.4) at coding-DNA position 1853, where G is replaced by A; at the protein level this means replaces arginine at residue 618 with lysine — a missense variant. Submitter rationale: The p.R618K variant (also known as c.1853G>A and 1972G>A), located in coding exon 9 of the BRCA1 gene, results from a G to A substitution at nucleotide position 1853. The arginine at codon 618 is replaced by lysine, an amino acid with highly similar properties. This variant was not reported in population based cohorts in the following databases: Database of Single Nucleotide Polymorphisms (dbSNP), NHLBI Exome Sequencing Project (ESP), and 1000 Genomes Project. In the ESP, this variant was not observed in 6502 samples (13004 alleles) with coverage at this position. To date, this alteration has been detected with an allele frequency of approximately 0.001% (greater than 150,000 alleles tested) in our clinical cohort. This amino acid position is well conserved in available vertebrate species. In addition, the in silico prediction for this alteration is inconclusive. Since supporting evidence is limited at this time, the clinical significance of p.R618K remains unclear.
