Uncertain significance for Hereditary cancer-predisposing syndrome — the classification assigned by Ambry Genetics to NM_000051.4(ATM):c.1019C>A (p.Ala340Asp), citing Ambry Variant Classification Scheme 2023. This variant lies in the ATM gene (transcript NM_000051.4) at coding-DNA position 1019, where C is replaced by A; at the protein level this means replaces alanine at residue 340 with aspartic acid — a missense variant. Submitter rationale: The p.A340D variant (also known as c.1019C>A), located in coding exon 7 of the ATM gene, results from a C to A substitution at nucleotide position 1019. The alanine at codon 340 is replaced by aspartic acid, an amino acid with dissimilar properties. This variant was not reported in population based cohorts in the following databases: Database of Single Nucleotide Polymorphisms (dbSNP), NHLBI Exome Sequencing Project (ESP), and 1000 Genomes Project. In the ESP, this variant was not observed in 6499 samples (12998 alleles) with coverage at this position. To date, this alteration has been detected with an allele frequency of approximately 0.002% (greater than 66000 alleles tested) in our clinical cohort. This amino acid position is highly conserved in available vertebrate species. In addition, this alteration is predicted to be possibly damaging and tolerated by PolyPhen and SIFT in silico analyses, respectively. Since supporting evidence is limited at this time, the clinical significance of p.A340D remains unclear.
