Uncertain significance for Hereditary cancer-predisposing syndrome — the classification assigned by Ambry Genetics to NM_000059.4(BRCA2):c.1858T>C (p.Phe620Leu), citing Ambry Variant Classification Scheme 2023. This variant lies in the BRCA2 gene (transcript NM_000059.4) at coding-DNA position 1858, where T is replaced by C; at the protein level this means replaces phenylalanine at residue 620 with leucine — a missense variant. Submitter rationale: The p.F620L variant (also known as c.1858T>C), located in coding exon 9 of the BRCA2 gene, results from a T to C substitution at nucleotide position 1858. The phenylalanine at codon 620 is replaced by leucine, an amino acid with highly similar properties. This variant was not reported in population based cohorts in the following databases: Database of Single Nucleotide Polymorphisms (dbSNP), NHLBI Exome Sequencing Project (ESP), and 1000 Genomes Project. In the ESP, this variant was not observed in 6465 samples (12930 alleles) with coverage at this position. To date, this alteration has been detected with an allele frequency of approximately 0.001% (greater than 150000 alleles tested) in our clinical cohort. This amino acid position is poorly conserved in available vertebrate species. In addition, this alteration is predicted to be tolerated by in silico analysis. Since supporting evidence is limited at this time, the clinical significance of p.F620L remains unclear.
