Uncertain significance for Hereditary cancer-predisposing syndrome — the classification assigned by Ambry Genetics to NM_000051.4(ATM):c.5263A>G (p.Met1755Val), citing Ambry Variant Classification Scheme 2023: The p.M1755V variant (also known as c.5263A>G), located in coding exon 34 of the ATM gene, results from an A to G substitution at nucleotide position 5263. The methionine at codon 1755 is replaced by valine, an amino acid with highly similar properties. This alteration has been detected in a cohort of 122 patients who underwent multi-gene panel testing for hereditary cancer after having previously tested negative for mutations in BRCA1 and BRCA2 (Yadav S et al. Fam Cancer, 2017 07;16:319-328). Additionally, this alteration was detected in 1/5589 German BRCA1/2-negative probands with breast cancer (Hauke J et al. Cancer Med, 2018 04;7:1349-1358). This amino acid position is poorly conserved in available vertebrate species. In addition, this alteration is predicted to be tolerated by in silico analysis. Since supporting evidence is limited at this time, the clinical significance of this alteration remains unclear.
