Uncertain significance for Hereditary cancer-predisposing syndrome — the classification assigned by Ambry Genetics to NM_000465.4(BARD1):c.1318G>T (p.Asp440Tyr), citing Ambry Variant Classification Scheme 2023. This variant lies in the BARD1 gene (transcript NM_000465.4) at coding-DNA position 1318, where G is replaced by T; at the protein level this means replaces aspartic acid at residue 440 with tyrosine — a missense variant. Submitter rationale: The p.D440Y variant (also known as c.1318G>T), located in coding exon 5 of the BARD1 gene, results from a G to T substitution at nucleotide position 1318. The aspartic acid at codon 440 is replaced by tyrosine, an amino acid with highly dissimilar properties. This variant was not reported in population based cohorts in the following databases: Database of Single Nucleotide Polymorphisms (dbSNP), NHLBI Exome Sequencing Project (ESP), and 1000 Genomes Project. In the ESP, this variant was not observed in 6503 samples (13006 alleles) with coverage at this position. To date, this alteration has been detected with an allele frequency of approximately 0.003% (greater than 40000 alleles tested) in our clinical cohort. This amino acid position is highly conserved in available vertebrate species. In addition, this alteration is predicted to be deleterious by in silico analysis. Since supporting evidence is limited at this time, the clinical significance of p.D440Y remains unclear.
