Uncertain significance for Hereditary cancer-predisposing syndrome — the classification assigned by Ambry Genetics to NM_000051.4(ATM):c.8761A>T (p.Thr2921Ser), citing Ambry Variant Classification Scheme 2023. This variant lies in the ATM gene (transcript NM_000051.4) at coding-DNA position 8761, where A is replaced by T; at the protein level this means replaces threonine at residue 2921 with serine — a missense variant. Submitter rationale: The p.T2921S variant (also known as c.8761A>T), located in coding exon 59 of the ATM gene, results from an A to T substitution at nucleotide position 8761. The threonine at codon 2921 is replaced by serine, an amino acid with similar properties. This variant was not reported in population based cohorts in the following databases: Database of Single Nucleotide Polymorphisms (dbSNP), NHLBI Exome Sequencing Project (ESP), and 1000 Genomes Project. In the ESP, this variant was not observed in 6499 samples (12998 alleles) with coverage at this position. To date, this alteration has been detected with an allele frequency of approximately 0.002% (greater than 41000 alleles tested) in our clinical cohort. This amino acid position is highly conserved in available vertebrate species. In addition, this alteration is predicted to be possibly damaging and deleterious by PolyPhen and SIFT in silico analyses, respectively. Since supporting evidence is limited at this time, the clinical significance of p.T2921S remains unclear.
