NM_000465.4(BARD1):c.823T>A (p.Leu275Ile) was classified as Uncertain significance for Hereditary cancer-predisposing syndrome by Ambry Genetics, citing Ambry Variant Classification Scheme 2023. This variant lies in the BARD1 gene (transcript NM_000465.4) at coding-DNA position 823, where T is replaced by A; at the protein level this means replaces leucine at residue 275 with isoleucine — a missense variant. Submitter rationale: The p.L275I variant (also known as c.823T>A), located in coding exon 4 of the BARD1 gene, results from a T to A substitution at nucleotide position 823. The leucine at codon 275 is replaced by isoleucine, an amino acid with highly similar properties. This variant was not reported in population based cohorts in the following databases: Database of Single Nucleotide Polymorphisms (dbSNP), NHLBI Exome Sequencing Project (ESP), and 1000 Genomes Project. In the ESP, this variant was not observed in 6501 samples (13002 alleles) with coverage at this position. To date, this alteration has been detected with an allele frequency of approximately 0.003% (greater than 40000 alleles tested) in our clinical cohort. This amino acid position is not well conserved in available vertebrate species. In addition, this alteration is predicted to be tolerated by in silico analysis. Since supporting evidence is limited at this time, the clinical significance of p.L275I remains unclear.
