NM_005359.6(SMAD4):c.565C>G (p.Arg189Gly) was classified as Uncertain significance for Hereditary cancer-predisposing syndrome; Familial thoracic aortic aneurysm and aortic dissection by Ambry Genetics, citing Ambry Variant Classification Scheme 2023: The p.R189G variant (also known as c.565C>G), located in coding exon 4 of the SMAD4 gene, results from a C to G substitution at nucleotide position 565. The arginine at codon 189 is replaced by glycine, an amino acid with dissimilar properties. This variant was not reported in population based cohorts in the following databases: Database of Single Nucleotide Polymorphisms (dbSNP), NHLBI Exome Sequencing Project (ESP), and 1000 Genomes Project. In the ESP, this variant was not observed in 6503 samples (13006 alleles) with coverage at this position. To date, this alteration has been detected with an allele frequency of approximately 0.01% (greater than 30000 alleles tested) in our clinical cohort. This amino acid position is well conserved through mammals. In addition, the in silico prediction for this alteration is inconclusive. Since supporting evidence is limited at this time, the clinical significance of p.R189G remains unclear.
