Uncertain significance for Hereditary cancer-predisposing syndrome — the classification assigned by Ambry Genetics to NM_000249.4(MLH1):c.1250C>T (p.Thr417Ile), citing Ambry Variant Classification Scheme 2023. This variant lies in the MLH1 gene (transcript NM_000249.4) at coding-DNA position 1250, where C is replaced by T; at the protein level this means replaces threonine at residue 417 with isoleucine — a missense variant. Submitter rationale: The p.T417I variant (also known as c.1250C>T), located in coding exon 12 of the MLH1 gene, results from a C to T substitution at nucleotide position 1250. The threonine at codon 417 is replaced by isoleucine, an amino acid with similar properties. This variant was not reported in population based cohorts in the following databases: Database of Single Nucleotide Polymorphisms (dbSNP), NHLBI Exome Sequencing Project (ESP), and 1000 Genomes Project. In the ESP, this variant was not observed in 6503 samples (13006 alleles) with coverage at this position. To date, this alteration has been detected with an allele frequency of approximately 0.002% (greater than 42000 alleles tested) in our clinical cohort. This amino acid position is not well conserved in available vertebrate species. In addition, the in silico prediction for this alteration is inconclusive. Since supporting evidence is limited at this time, the clinical significance of p.T417I remains unclear.
