Uncertain significance for Hereditary cancer-predisposing syndrome — the classification assigned by Ambry Genetics to NM_002878.4(RAD51D):c.164G>A (p.Arg55Gln), citing Ambry Variant Classification Scheme 2023. This variant lies in the RAD51D gene (transcript NM_002878.4) at coding-DNA position 164, where G is replaced by A; at the protein level this means replaces arginine at residue 55 with glutamine — a missense variant. Submitter rationale: The p.R55Q variant (also known as c.164G>A), located in coding exon 3 of the RAD51D gene, results from a G to A substitution at nucleotide position 164. The arginine at codon 55 is replaced by glutamine, an amino acid with highly similar properties. This alteration has been reported in 1/1197 individuals from Greece, Romania, and Turkey undergoing evaluation for inherited cancer predisposition (Tsaousis GN et al. BMC Cancer, 2019 Jun;19:535) and in 1/3251 individuals who met eligibility criteria for hereditary breast and ovarian cancer syndrome (Lerner-Ellis J et al. J Cancer Res Clin Oncol, 2021 Mar;147:871-879). This variant was also identified in a cohort of 3,579 African males diagnosed with prostate cancer who underwent multi-gene panel testing of 19 DNA repair and cancer predisposition genes (Matejcic M et al. JCO Precis Oncol, 2020 Jan;4:32-43). This amino acid position is highly conserved in available vertebrate species. In addition, the in silico prediction for this alteration is inconclusive. Based on the available evidence, the clinical significance of this variant remains unclear.
