Uncertain significance — the classification assigned by GeneDx to NM_000465.4(BARD1):c.61C>T (p.Arg21Cys), citing GeneDx Variant Classification (06012015). This variant lies in the BARD1 gene (transcript NM_000465.4) at coding-DNA position 61, where C is replaced by T; at the protein level this means replaces arginine at residue 21 with cysteine — a missense variant. Submitter rationale: This variant is denoted BARD1 c.61C>T at the cDNA level, p.Arg21Cys (R21C) at the protein level, and results in the change of an Arginine to a Cysteine (CGT>TGT). This variant has not, to our knowledge, been published in the literature as a germline variant; however, it has been reported in a lung tumor (Imielinski 2012). BARD1 Arg21Cys was not observed in large population cohorts (NHLBI Exome Sequencing Project, The 1000 Genomes Consortium 2015, Lek 2016). Since Arginine and Cysteine differ in polarity, charge, size or other properties, this is considered a non-conservative amino acid substitution. BARD1 Arg21Cys occurs at a position that is not conserved and is not located in a known functional domain. In silico analyses predict that this variant is unlikely to alter protein structure or function. Based on currently available evidence, it is unclear whether BARD1 Arg21Cys is a pathogenic or benign variant. We consider it to be a variant of uncertain significance.
