NM_004369.4(COL6A3):c.6221G>A (p.Gly2074Asp) was classified as Pathogenic for Bethlem myopathy 1A by Labcorp Genetics (formerly Invitae), Labcorp, citing Invitae Variant Classification Sherloc (09022015). This variant lies in the COL6A3 gene (transcript NM_004369.4) at coding-DNA position 6221, where G is replaced by A; at the protein level this means replaces glycine at residue 2074 with aspartic acid — a missense variant. Submitter rationale: This sequence change replaces glycine, which is neutral and non-polar, with aspartic acid, which is acidic and polar, at codon 2074 of the COL6A3 protein (p.Gly2074Asp). This variant is not present in population databases (gnomAD no frequency). This missense change has been observed in individual(s) with autosomal dominant Ullrich congenital muscular dystrophy (PMID: 18825676, 37023487). In at least one individual the variant was observed to be de novo. ClinVar contains an entry for this variant (Variation ID: 2203291). Invitae Evidence Modeling of protein sequence and biophysical properties (such as structural, functional, and spatial information, amino acid conservation, physicochemical variation, residue mobility, and thermodynamic stability) indicates that this missense variant is expected to disrupt COL6A3 protein function with a positive predictive value of 80%. This variant disrupts the triple helix domain of COL6A3. Glycine residues within the Gly-Xaa-Yaa repeats of the triple helix domain are required for the structure and stability of fibrillar collagens (PMID: 7695699, 8218237, 19344236). In COL6A3, missense variants at these glycine residues are significantly enriched in individuals with autosomal dominant disease (PMID: 15689448, 24038877) compared to the general population (ExAC). This variant disrupts the p.Gly2074 amino acid residue in COL6A3. Other variant(s) that disrupt this residue have been observed in individuals with COL6A3-related conditions (PMID: 20976770, 28831785), which suggests that this may be a clinically significant amino acid residue. For these reasons, this variant has been classified as Pathogenic.
