Likely pathogenic — the classification assigned by Labcorp Genetics (formerly Invitae), Labcorp to NM_000091.5(COL4A3):c.4028-27A>G, citing Invitae Variant Classification Sherloc (09022015): This sequence change falls in intron 45 of the COL4A3 gene. It does not directly change the encoded amino acid sequence of the COL4A3 protein. RNA analysis indicates that this variant induces altered splicing and likely results in a shortened protein product. This variant is not present in population databases (gnomAD no frequency). This variant has been observed in individual(s) with autosomal recessive Alport syndrome (PMID: 24633401). In at least one individual the data is consistent with being in trans (on the opposite chromosome) from a pathogenic variant. ClinVar contains an entry for this variant (Variation ID: 2203283). Studies have shown that this variant results in skipping of exon 46, but is expected to preserve the integrity of the reading-frame (PMID: 24633401). In summary, the currently available evidence indicates that the variant is pathogenic, but additional data are needed to prove that conclusively. Therefore, this variant has been classified as Likely Pathogenic.
