Uncertain significance for 3-hydroxy-3-methylglutaryl-CoA synthase deficiency — the classification assigned by Labcorp Genetics (formerly Invitae), Labcorp to NM_005518.4(HMGCS2):c.797T>C (p.Leu266Ser), citing Invitae Variant Classification Sherloc (09022015): Algorithms developed to predict the effect of missense changes on protein structure and function (SIFT, PolyPhen-2, Align-GVGD) all suggest that this variant is likely to be disruptive. This sequence change replaces leucine, which is neutral and non-polar, with serine, which is neutral and polar, at codon 266 of the HMGCS2 protein (p.Leu266Ser). The frequency data for this variant in the population databases is considered unreliable, as metrics indicate poor data quality at this position in the gnomAD database. This missense change has been observed in individual(s) with HMGCS2 deficiency (PMID: 25511235). Experimental studies have shown that this missense change affects HMGCS2 function (PMID: 29597274).
