Uncertain significance — the classification assigned by Labcorp Genetics (formerly Invitae), Labcorp to NM_033380.3(COL4A5):c.58T>C (p.Trp20Arg), citing Invitae Variant Classification Sherloc (09022015): This sequence change replaces tryptophan with arginine at codon 20 of the COL4A5 protein (p.Trp20Arg). The tryptophan residue is weakly conserved and there is a moderate physicochemical difference between tryptophan and arginine. This variant is not present in population databases (ExAC no frequency). This variant has not been reported in the literature in individuals affected with COL4A5-related conditions. Advanced modeling of protein sequence and biophysical properties (such as structural, functional, and spatial information, amino acid conservation, physicochemical variation, residue mobility, and thermodynamic stability) performed at Invitae indicates that this missense variant is not expected to disrupt COL4A5 protein function. In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance.
