NM_001283009.2(RTEL1):c.304T>C (p.Cys102Arg) was classified as Uncertain significance for Dyskeratosis congenita, autosomal recessive 5; Pulmonary fibrosis and/or bone marrow failure, Telomere-related, 3 by Labcorp Genetics (formerly Invitae), Labcorp, citing Invitae Variant Classification Sherloc (09022015). This variant lies in the RTEL1 gene (transcript NM_001283009.2) at coding-DNA position 304, where T is replaced by C; at the protein level this means replaces cysteine at residue 102 with arginine — a missense variant. Submitter rationale: This sequence change replaces cysteine with arginine at codon 102 of the RTEL1 protein (p.Cys102Arg). The cysteine residue is weakly conserved and there is a large physicochemical difference between cysteine and arginine. This variant is present in population databases (rs754769101, ExAC 0.01%). This variant has not been reported in the literature in individuals affected with RTEL1-related conditions. Algorithms developed to predict the effect of missense changes on protein structure and function (SIFT, PolyPhen-2, Align-GVGD) all suggest that this variant is likely to be tolerated. In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance.
