Uncertain significance for Jeune thoracic dystrophy; Nephronophthisis — the classification assigned by Labcorp Genetics (formerly Invitae), Labcorp to NM_024753.5(TTC21B):c.2167C>T (p.Arg723Trp), citing Invitae Variant Classification Sherloc (09022015): This sequence change replaces arginine with tryptophan at codon 723 of the TTC21B protein (p.Arg723Trp). The arginine residue is moderately conserved and there is a moderate physicochemical difference between arginine and tryptophan. This variant is present in population databases (rs201500142, ExAC 0.002%) but has not been reported in the literature in individuals with a TTC21B-related disease. ClinVar contains an entry for this variant (Variation ID: 216766). Algorithms developed to predict the effect of missense changes on protein structure and function do not agree on the potential impact of this missense change (SIFT: "Deleterious"; PolyPhen-2: "Possibly Damaging"; Align-GVGD: "Class C0"). In summary, this variant is a rare missense change with uncertain impact on protein function. There is no indication that it causes disease, but the available evidence is currently insufficient to prove that conclusively. Therefore, it has been classified as a Variant of Uncertain Significance.
