Likely pathogenic — the classification assigned by GeneDx to NM_001040436.3(YARS2):c.374G>C (p.Arg125Pro), citing GeneDx Variant Classification (06012015): p.Arg125Pro (CGT>CCT): c.374 G>C in exon 1 of the YARS2 gene (NM_001040436.2) The R125P variant has not been published as a mutation, nor has it been reported as a benign polymorphism to our knowledge. The R125P variant is a non-conservative amino acid substitution, as these residues differ in polarity, charge, size and/or other properties and is more likely to impact secondary structure. This substitution occurs at a highly conserved position in the YARS2 protein, and multiple in-silico analysis programs predict that R125P is damaging to the YARS2 protein. Therefore, R125P is a strong candidate for a disease-causing mutation, however the possibility that it is a benign variant cannot be excluded. The variant is found in MITONUC-MITOP panel(s).
