Uncertain significance — the classification assigned by Women's Health and Genetics/Laboratory Corporation of America, LabCorp to NM_001182.5(ALDH7A1):c.1A>G (p.Met1Val), citing LabCorp Variant Classification Summary - May 2015. This variant lies in the ALDH7A1 gene (transcript NM_001182.5) at coding-DNA position 1, where A is replaced by G; at the protein level this means replaces methionine at residue 1 with valine — a missense variant. Submitter rationale: Variant summary: ALDH7A1 c.1A>G (p.Met1Val) alters the initiation codon and is predicted to result either in absence of the protein or truncation of the encoded protein due to translation initiation at a downstream codon. An alternative downstream in-frame start codon (Met29) is located in the encoded protein. An activation of potential downstream translation initiation at this site would result in a shortened protein missing the first 28 amino acids from the protein sequence. To our knowledge at least one other transcript (NM_001201377) uses this alternative start codon and no pathogenic variants have been reported in the region upstream of Met29. Two of three in-silico tools predict a benign effect of the variant on protein function. The variant was absent in 155310 control chromosomes (gnomAD). The available data on variant occurrences in the general population are insufficient to allow any conclusion about variant significance. To our knowledge, no occurrence of c.1A>G in individuals affected with Pyridoxine-Dependent Epilepsy and no experimental evidence demonstrating its impact on protein function have been reported. One clinical diagnostic laboratory has submitted a clinical-significance assessment for this variant to ClinVar after 2014 and classified the variant as uncertain significance. Based on the evidence outlined above, the variant was classified as uncertain significance.
