Uncertain significance for Mucopolysaccharidosis type 1 — the classification assigned by Labcorp Genetics (formerly Invitae), Labcorp to NM_000203.5(IDUA):c.1664G>A (p.Arg555His), citing Invitae Variant Classification Sherloc (09022015): This sequence change replaces arginine, which is basic and polar, with histidine, which is basic and polar, at codon 555 of the IDUA protein (p.Arg555His). The frequency data for this variant in the population databases is considered unreliable, as metrics indicate poor data quality at this position in the gnomAD database. This missense change has been observed in individual(s) with IDUA-related conditions (PMID: 29843745). In at least one individual the data is consistent with being in trans (on the opposite chromosome) from a pathogenic variant. Advanced modeling of protein sequence and biophysical properties (such as structural, functional, and spatial information, amino acid conservation, physicochemical variation, residue mobility, and thermodynamic stability) performed at Invitae indicates that this missense variant is not expected to disrupt IDUA protein function. In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance.
