Uncertain significance for Neonatal encephalomyopathy-cardiomyopathy-respiratory distress syndrome — the classification assigned by Labcorp Genetics (formerly Invitae), Labcorp to NM_016035.5(COQ4):c.262C>G (p.Gln88Glu), citing Invitae Variant Classification Sherloc (09022015). This variant lies in the COQ4 gene (transcript NM_016035.5) at coding-DNA position 262, where C is replaced by G; at the protein level this means replaces glutamine at residue 88 with glutamic acid — a missense variant. Submitter rationale: This sequence change replaces glutamine, which is neutral and polar, with glutamic acid, which is acidic and polar, at codon 88 of the COQ4 protein (p.Gln88Glu). This variant is present in population databases (rs201212305, gnomAD 0.03%). This variant has not been reported in the literature in individuals affected with COQ4-related conditions. ClinVar contains an entry for this variant (Variation ID: 2151739). Advanced modeling of protein sequence and biophysical properties (such as structural, functional, and spatial information, amino acid conservation, physicochemical variation, residue mobility, and thermodynamic stability) has been performed at Invitae for this missense variant, however the output from this modeling did not meet the statistical confidence thresholds required to predict the impact of this variant on COQ4 protein function. In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance.
