This variant lies in the BTK gene (transcript NM_000061.3) at the canonical splice donor site of the intron immediately after coding-DNA position 309, where G is replaced by A; at the protein level this means a change at this position may disrupt normal splicing. Submitter rationale: This sequence change affects a donor splice site in intron 4 of the BTK gene. RNA analysis indicates that disruption of this splice site induces altered splicing and may result in an absent or disrupted protein product. This variant is not present in population databases (gnomAD no frequency). Disruption of this splice site has been observed in individuals with clinical features of X-linked agammaglobulinaemia (PMID: 7633420; Invitae). This variant is also known as c.441+1G>A. Studies have shown that disruption of this splice site results in skipping of exon 5 and introduces a premature termination codon (PMID: 7633420). The resulting mRNA is expected to undergo nonsense-mediated decay. For these reasons, this variant has been classified as Pathogenic.
