Likely pathogenic for HYPERHOMOCYSTEINEMIA, THROMBOTIC, CBS-RELATED — the classification assigned by Labcorp Genetics (formerly Invitae), Labcorp to NM_000071.3(CBS):c.1361T>A (p.Val454Glu), citing Invitae Variant Classification Sherloc (09022015): This variant is not present in population databases (gnomAD no frequency). This sequence change replaces valine, which is neutral and non-polar, with glutamic acid, which is acidic and polar, at codon 454 of the CBS protein (p.Val454Glu). In summary, the currently available evidence indicates that the variant is pathogenic, but additional data are needed to prove that conclusively. Therefore, this variant has been classified as Likely Pathogenic. Experimental studies have shown that this missense change affects CBS function (PMID: 8528202). Algorithms developed to predict the effect of missense changes on protein structure and function (SIFT, PolyPhen-2, Align-GVGD) all suggest that this variant is likely to be tolerated. This variant is also known as T 1364 A (V455 E). This missense change has been observed in individual(s) with cystathionine beta-synthase (CBS) deficiency (PMID: 8528202). In at least one individual the data is consistent with being in trans (on the opposite chromosome) from a pathogenic variant.
