Pathogenic for Cardiovascular phenotype; Hereditary cancer-predisposing syndrome — the classification assigned by Ambry Genetics to NM_001042492.3(NF1):c.6557C>A (p.Ser2186Ter), citing Ambry Variant Classification Scheme 2023. This variant lies in the NF1 gene (transcript NM_001042492.3) at coding-DNA position 6557, where C is replaced by A; at the protein level this means converts the codon for serine at residue 2186 into a premature stop signal — a nonsense variant expected to truncate the protein. Submitter rationale: The p.S2165* pathogenic mutation (also known as c.6494C>A), located in coding exon 42 of the NF1 gene, results from a C to A substitution at nucleotide position 6494. This changes the amino acid from a serine to a stop codon within coding exon 42. This alteration was observed in 1/77 Taiwanese/Chinese patients meeting diagnostic criteria for NF1 (Lee MJ et al. Hum Mutat, 2006 Aug;27:832). This variant is considered to be rare based on population cohorts in the Genome Aggregation Database (gnomAD). In addition to the clinical data presented in the literature, this alteration is expected to result in loss of function by premature protein truncation or nonsense-mediated mRNA decay. As such, this alteration is interpreted as a disease-causing mutation.
