This variant lies in the PRF1 gene (transcript NM_001083116.3) at coding-DNA position 1034, where C is replaced by G; at the protein level this means replaces proline at residue 345 with arginine — a missense variant. Submitter rationale: This sequence change replaces proline, which is neutral and non-polar, with arginine, which is basic and polar, at codon 345 of the PRF1 protein (p.Pro345Arg). This variant is not present in population databases (gnomAD no frequency). This missense change has been observed in individual(s) with familial haemophagocytic lymphohistiocytosis (PMID: 28750028; internal data). In at least one individual the data is consistent with being in trans (on the opposite chromosome) from a pathogenic variant. It has also been observed to segregate with disease in related individuals. ClinVar contains an entry for this variant (Variation ID: 2136870). Invitae Evidence Modeling of protein sequence and biophysical properties (such as structural, functional, and spatial information, amino acid conservation, physicochemical variation, residue mobility, and thermodynamic stability) indicates that this missense variant is expected to disrupt PRF1 protein function with a positive predictive value of 95%.
