Pathogenic for Hereditary hyperekplexia — the classification assigned by Labcorp Genetics (formerly Invitae), Labcorp to NM_000171.4(GLRA1):c.278G>T (p.Arg93Leu), citing Invitae Variant Classification Sherloc (09022015): This sequence change replaces arginine, which is basic and polar, with leucine, which is neutral and non-polar, at codon 93 of the GLRA1 protein (p.Arg93Leu). This variant is not present in population databases (gnomAD no frequency). This missense change has been observed in individual(s) with autosomal recessive hyperekplexia (PMID: 20631190). In at least one individual the data is consistent with being in trans (on the opposite chromosome) from a pathogenic variant. This variant is also known as R65L. Advanced modeling of protein sequence and biophysical properties (such as structural, functional, and spatial information, amino acid conservation, physicochemical variation, residue mobility, and thermodynamic stability) performed at Invitae indicates that this missense variant is expected to disrupt GLRA1 protein function. Experimental studies have shown that this missense change affects GLRA1 function (PMID: 20631190). This variant disrupts the p.Arg93 amino acid residue in GLRA1. Other variant(s) that disrupt this residue have been determined to be pathogenic (PMID: 20631190, 24108130). This suggests that this residue is clinically significant, and that variants that disrupt this residue are likely to be disease-causing. For these reasons, this variant has been classified as Pathogenic.
