NM_005359.6(SMAD4):c.425-9A>G was classified as Pathogenic for Juvenile polyposis syndrome by Labcorp Genetics (formerly Invitae), Labcorp, citing Invitae Variant Classification Sherloc (09022015). This variant lies in the SMAD4 gene (transcript NM_005359.6) at 9 bases into the intron immediately before coding-DNA position 425, where A is replaced by G. Submitter rationale: ClinVar contains an entry for this variant (Variation ID: 2134379). For these reasons, this variant has been classified as Pathogenic. Studies have shown that this variant results in activation of a cryptic splice site and introduces a premature termination codon (Invitae). The resulting mRNA is expected to undergo nonsense-mediated decay. This variant has been observed in individual(s) with clinical features of SMAD4-related conditions (Invitae). In at least one individual the variant was observed to be de novo. This variant is not present in population databases (gnomAD no frequency). This sequence change falls in intron 3 of the SMAD4 gene. It does not directly change the encoded amino acid sequence of the SMAD4 protein. RNA analysis indicates that this variant induces altered splicing and may result in an absent or disrupted protein product.
