Uncertain significance for Hypertrophic cardiomyopathy — the classification assigned by Labcorp Genetics (formerly Invitae), Labcorp to NM_000257.4(MYH7):c.2242_2243delinsAT (p.Ser748Ile), citing Invitae Variant Classification Sherloc (09022015): This sequence change replaces serine, which is neutral and polar, with isoleucine, which is neutral and non-polar, at codon 748 of the MYH7 protein (p.Ser748Ile). Information on the frequency of this variant in the gnomAD database is not available, as this variant may be reported differently in the database. This missense change has been observed in individuals with clinical features of MYH7-related conditions (Invitae). ClinVar contains an entry for this variant (Variation ID: 2123150). An algorithm developed to predict the effect of missense changes on protein structure and function (PolyPhen-2) suggests that this variant is likely to be disruptive. This variant is found within a region of MYH7 between codons 181 and 937 that contains the majority of the myosin head domain. Missense variants in this region have been shown to be significantly overrepresented in individuals with hypertrophic cardiomyopathy (PMID: 27532257). In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance.
