Uncertain significance for Wilms tumor 1 — the classification assigned by Labcorp Genetics (formerly Invitae), Labcorp to NM_004484.4(GPC3):c.1394A>G (p.Lys465Arg), citing Invitae Variant Classification Sherloc (09022015). This variant lies in the GPC3 gene (transcript NM_004484.4) at coding-DNA position 1394, where A is replaced by G; at the protein level this means replaces lysine at residue 465 with arginine — a missense variant. Submitter rationale: This sequence change replaces lysine, which is basic and polar, with arginine, which is basic and polar, at codon 465 of the GPC3 protein (p.Lys465Arg). This variant is present in population databases (no rsID available, gnomAD 0.001%), including at least one homozygous and/or hemizygous individual. This variant has not been reported in the literature in individuals affected with GPC3-related conditions. Advanced modeling of protein sequence and biophysical properties (such as structural, functional, and spatial information, amino acid conservation, physicochemical variation, residue mobility, and thermodynamic stability) has been performed at Invitae for this missense variant, however the output from this modeling did not meet the statistical confidence thresholds required to predict the impact of this variant on GPC3 protein function. In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance.
