Uncertain significance for Early-infantile DEE — the classification assigned by Labcorp Genetics (formerly Invitae), Labcorp to NM_021072.4(HCN1):c.1309C>G (p.His437Asp), citing Invitae Variant Classification Sherloc (09022015): This sequence change replaces histidine, which is basic and polar, with aspartic acid, which is acidic and polar, at codon 437 of the HCN1 protein (p.His437Asp). This variant is not present in population databases (gnomAD no frequency). This variant has not been reported in the literature in individuals affected with HCN1-related conditions. This missense change has been observed in at least one individual who was not affected with HCN1-related conditions (Invitae). ClinVar contains an entry for this variant (Variation ID: 2099551). Advanced modeling of protein sequence and biophysical properties (such as structural, functional, and spatial information, amino acid conservation, physicochemical variation, residue mobility, and thermodynamic stability) performed at Invitae indicates that this missense variant is expected to disrupt HCN1 protein function with a positive predictive value of 80%. In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance.
