This variant lies in the CLCNKB gene (transcript NM_000085.5) at the canonical splice donor site of the intron immediately after coding-DNA position 498, where T is replaced by G; at the protein level this means a change at this position may disrupt normal splicing. Submitter rationale: This sequence change affects a donor splice site in intron 5 of the CLCNKB gene. It is expected to disrupt RNA splicing. Variants that disrupt the donor or acceptor splice site typically lead to a loss of protein function (PMID: 16199547), and loss-of-function variants in CLCNKB are known to be pathogenic (PMID: 24830959, 26920127, 28381550, 29254190). For these reasons, this variant has been classified as Pathogenic. Algorithms developed to predict the effect of sequence changes on RNA splicing suggest that this variant may disrupt the consensus splice site. Disruption of this splice site has been observed in individual(s) with Bartter syndrome (PMID: 28381550, 31672324). In at least one individual the data is consistent with being in trans (on the opposite chromosome) from a pathogenic variant. This variant is not present in population databases (gnomAD no frequency).
