This variant lies in the RAPSN gene (transcript NM_005055.5) at coding-DNA position 1168, deleting one base; at the protein level this means shifts the reading frame starting at cysteine residue 390, producing a truncated or aberrant protein — a frameshift variant. Submitter rationale: For these reasons, this variant has been classified as Pathogenic. This variant disrupts a region of the RAPSN protein in which other variant(s) (p.Thr396Profs*12) have been determined to be pathogenic (PMID: 26782015, 29054425). This suggests that this is a clinically significant region of the protein, and that variants that disrupt it are likely to be disease-causing. This variant has not been reported in the literature in individuals affected with RAPSN-related conditions. This variant is not present in population databases (gnomAD no frequency). This sequence change creates a premature translational stop signal (p.Cys390Alafs*18) in the RAPSN gene. While this is not anticipated to result in nonsense mediated decay, it is expected to disrupt the last 23 amino acid(s) of the RAPSN protein.
