Uncertain significance — the classification assigned by Labcorp Genetics (formerly Invitae), Labcorp to NM_000498.3(CYP11B2):c.1211A>G (p.Gln404Arg), citing Invitae Variant Classification Sherloc (09022015): This sequence change replaces glutamine, which is neutral and polar, with arginine, which is basic and polar, at codon 404 of the CYP11B2 protein (p.Gln404Arg). This variant is present in population databases (rs139367087, gnomAD 0.1%). This variant has not been reported in the literature in individuals affected with CYP11B2-related conditions. Advanced modeling of protein sequence and biophysical properties (such as structural, functional, and spatial information, amino acid conservation, physicochemical variation, residue mobility, and thermodynamic stability) performed at Invitae indicates that this missense variant is not expected to disrupt CYP11B2 protein function. Experimental studies have shown that this missense change affects CYP11B2 function (PMID: 18710464). Algorithms developed to predict the effect of sequence changes on RNA splicing suggest that this variant may create or strengthen a splice site. In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance.
