This variant lies in the RYR1 gene (transcript NM_000540.3) at coding-DNA position 1598, where G is replaced by T; at the protein level this means replaces arginine at residue 533 with leucine — a missense variant. Submitter rationale: This missense variant replaces arginine with leucine at codon 533 of the RYR1 protein. Computational prediction suggests that this variant may have deleterious impact on protein structure and function (internally defined REVEL score threshold >= 0.7, PMID: 27666373). To our knowledge, functional studies have not been reported for this variant. This variant has not been reported in individuals affected with RYR1-related disorders in the literature. This variant has not been identified in the general population by the Genome Aggregation Database (gnomAD). A different variant occurring at the same codon, p.Arg533Cys, is known to cause malignant hyperthermia susceptibility (ClinVar variation ID: 133102), indicating that arginine at this position is important for RYR1 function. Although there is a suspicion for a pathogenic role, the available evidence is insufficient to determine the role of this variant in disease conclusively. Therefore, this variant is classified as a Variant of Uncertain Significance for autosomal dominant malignant hyperthermia susceptibility.
