This variant lies in the MBD5 gene (transcript NM_001378120.1) at coding-DNA position 644, where G is replaced by A; at the protein level this means replaces arginine at residue 215 with histidine — a missense variant. Submitter rationale: p.Arg215His (CGT>CAT): c.644 G>A in exon 9 of the MBD5 gene (NM_018328.4) The Arg215His variant has not been published as a mutation, nor has it been reported as a benign polymorphism to our knowledge. It was not observed in approximately 6,500 individuals of European and African American ancestry in the NHLBI Exome Sequencing Project, indicating it is not a common benign variant in these populations. The Arg215His variant is a conservative amino acid substitution, which is not likely to impact secondary protein structure as these residues share similar properties. This substitution occurs at a position that is conserved across species; however, to our knowledge, only deletions and frameshift mutations in MBD5 have been published in association with epilepsy. In silico analysis is inconsistent in its predictions as to whether or not the variant is damaging to the protein structure/function. Therefore, based on the currently available information, it is unclear whether this variant is a pathogenic mutation or a rare benign variant. The variant is found in INFANT-EPI panel(s).
