NM_000834.5(GRIN2B):c.1745C>G (p.Pro582Arg) was classified as Uncertain significance by GeneDx, citing GeneDx Variant Classification (06012015). This variant lies in the GRIN2B gene (transcript NM_000834.5) at coding-DNA position 1745, where C is replaced by G; at the protein level this means replaces proline at residue 582 with arginine — a missense variant. Submitter rationale: p.Pro582Arg (CCT>CGT): c.1745 C>G in exon 8 of the GRIN2B gene (NM_000834.3). The P582R variant has not been published as a mutation, nor has it been reported as a benign polymorphism to our knowledge. It was not observed in approximately 6,500 individuals of European and African American ancestry in the NHLBI Exome Sequencing Project, indicating it is not a common benign variant in these populations. The P582R variant is a non-conservative amino acid substitution, which is likely to impact secondary protein structure as these residues differ in polarity, charge, size and/or other properties. This substitution occurs at a position that is conserved across species, and in silico analysis predicts this variant is probably damaging to the protein structure/function. However, missense mutations in nearby residues have not been reported. Therefore, based on the currently available information, it is unclear whether this variant is a pathogenic mutation or a rare benign variant. The variant is found in EPILEPSY panel(s).
