Uncertain significance for Primary open angle glaucoma; Glaucoma 1, open angle, E; Amyotrophic lateral sclerosis type 12 — the classification assigned by Labcorp Genetics (formerly Invitae), Labcorp to NM_001008212.2(OPTN):c.1636C>T (p.Gln546Ter), citing Invitae Variant Classification Sherloc (09022015). This variant lies in the OPTN gene (transcript NM_001008212.2) at coding-DNA position 1636, where C is replaced by T; at the protein level this means converts the codon for glutamine at residue 546 into a premature stop signal — a nonsense variant expected to truncate the protein. Submitter rationale: This variant has not been reported in the literature in individuals affected with OPTN-related conditions. This variant is not present in population databases (gnomAD no frequency). This sequence change creates a premature translational stop signal (p.Gln546*) in the OPTN gene. While this is not anticipated to result in nonsense mediated decay, it is expected to disrupt the last 32 amino acid(s) of the OPTN protein. Experimental studies and prediction algorithms are not available or were not evaluated, and the functional significance of this variant is currently unknown. In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance. Algorithms developed to predict the effect of sequence changes on RNA splicing suggest that this variant may create or strengthen a splice site.
